Comparison of Laparoscopic Assisted Vaginal Hysterectomy and Total Laparoscopic Hysterectomy

Abstract
|
|
|
Objectives: To compare operative outcomes for laparoscopic assisted vaginal hysterectomy (LAVH) with total laparoscopic
hysterectomy (TLH).
Methods: Twenty eight of LAVH cases and eighteen of TLH cases with benign diseases were performed from Jan
2008 to May 2010 at department of Obstetrics & Gynecology, Daegu Catholic University Medical Center. Stage
4 hysterectomies by laparoscopic hysterectomy classification of the Richardson were performed in the LAVH group.
Operation time, uterine weight and complications such as needs of transfusion or infection were recorded.
Results: Stage 4 LAVH and TLH were performed by one surgeon (J.H.Kim). There were no significant differences
in patient’s demographics (age, weight, BMI, previous cesarean section) between the two groups. The mean operation
time was 117±48.7 min for the stage 4 LAVH group and 139±56.9 min for the TLH group (P=0.175) and
the operation time was not significantly different between the two groups but revealed tendency that was longer
in the TLH group. The uterine weight was not significantly different between the two groups [379±199 gm (99
to 997 gm) for the stage 4 LAVH group, 427±343 gm (101 to 1266 gm) for the TLH group, P=0.597]. The weight
of the uterus was over 280 gm in 75% of the LAVH group and 61% of the TLH group. Hemoglobin drop was
not significantly different between the two groups (-1.2±0.97 g/dl for the stage 4 LAVH group, -1.4±1.1 g/dl
for the TLH group, P=0.530).
Conclusion: The operation time was not significantly different between the two groups, but revealed tendency that
was longer in the TLH group. This result may be suggestive that stage 4 LAVH is more effective than TLH.
